Phas3r appears as an collectible item on the Space 54 map. Once you start the level if you have the Randomazzo and also the VIII card (the one that looks like a tree with a green background), if you select that card it will attract the Phas3r plus some empty tombs and candelabradors. Take the Phas3r and evolve it until it reaches level 7. Achievement will pop as soon as you leave or finish the level.

I Used Queen Sigma, And Selected The "Mad Groove" Card.
- This Brought The Weapon Right To Me.
The Weapon Can Be Evolved With An "Empty Tome" At Level 5.
Also, The Weapon At Level 8.
Achievement Unlocks At The End Of The Run, Once You've Successfully Gotten The Weapon To At Least Level 7.

On the new map, Space 54, there is a Phas3r sitting on the ground just above where you start. Pick it up, then level it up to level 7 for this. Make sure you then evolve it for EXTRA: Photonstorm (5G).
