You can find floor chickens by breaking light sources, such as fires and candles. You don't need to collect all five in one run, since it's cumulative. At the end of the run where you collect the fifth chicken, this achievement will pop and you'll unlock the weapon 'Garlic'.

This must be done in one run. Floor chickens (horribly worded, I know) are the cooked chickens on a platter that sometimes drop from destroying light sources or from enemies rarely. They give you decent health when you pick them up. Pick up five in one run to unlock this achievement. This is another achievement that'll unlock without really trying for it.
