Just a note to anyone playing on windows, this didn't seem to unlock for me after getting 100k kills, I had to load the game up on console, start a run and then die and it popped

This is cumulative across all runs with all characters.
While 100,000 enemies may sound like a huge amount, after your first couple runs to get your bearings and know what you're doing (plus some upgrades), you'll be killing thousands (and even tens of thousands) of enemies per run, making this achievement one of the earlier ones you'll unlock, actually.
