The map in a hidden tree room North East of spawn, however to get to it you will need to first head straight East and enter the hedge maze. Follow the maze and continue North West until you come to a clearing, then hu g the trees to the left and enter through a hidden opening on the top of the trees (just walk into them until you find it)

I left a note on the other DLC-map Achievement; the two have the same bug, and the workaround is the same for both. The Achievement will pop after exiting the level with the map (collected that go or any time prior) on BOTH the Windows and Xbox versions. It doesn't matter which you do first (as long as you get the map while you're there); you then need to go to the other, enter the Lake Foscari map, then exit (die, pause and quit, finish the stage, doesn't matter). It should then pop instantly. I did this one Windows first, and no Achievement popped until I played on Xbox; the Mt. Moonspell Achievement I did on Xbox first, and no Achievement popped until I played it on Windows. (Apparently, according to others who found similar problems, playing on Cloud counts as playing on console.) Hopefully none of this matters before long with a patch fixing it, but until then, here you are.

Just a note for those who may be in a similar situation. I already had the Lake Foscari map in my collection before the dlc came out, I was able to get the achievement by doing a run on on Lake Foscari. Even though the map was not available as a pick up I still got the achievement at the end of the run.

Note: This achievement requires you to have purchased and installed the Tides of the Foscari DLC so that you can access the 'Lake Foscari' stage.
Load up that stage, preferably with Sigma, and now you need to follow the green arrow to where the map is. Easier said than done, though, as this stage can be a bit of a pain to navigate without a map. I've provided a screenshot below that shows the map, and my character icon is where the map is located. So, as you can see from the image, you need to first get on the north side of the river that runs through the map. You can either go through the caves to the east, through the garden maze to the north, or just off to the west and then over the bridges. Once you've crossed the river, head to that dense tree section where the arrow is pointing. To get inside the trees, travel along the northern end of them and hold down until you reach a hidden entrance that opens up inside the forest and makes things visible. From there, simply head down to the map to grab it. Feel free to quit out immediately after grabbing the map to unlock this achievement.
