Each weapon (top row) has a support item (bottom row) that allows the weapon to evolve.
To do so you have to have both items equipped and then max out the weapon, following this you open a chest which will evolve the weapon into a more powerful variant.
You must do this with 6 weapons in a single run.
Tips-
- Dont go for this straight away, the further into the game you unlock more items, can reroll, skip and bannish items which makes this far easier.
- Dont rush to pickup chests as soon as they drop, save them until you are ready for evolutions
- there is a grimoire found on the pause screen and main menu which show evolutions
- when upgrading/selecting try to prioritise the weapon. Its the weapon that needs to be maxxed NOT the item
- when selecting/upgrading the choices will show any evo matches for items you already have equipped
- avoid evos that need at least 3 parts (eg guns)
- easiest level to focus on is stage 2, library
- if you really are struggling then late game you can play an endless mode which will give you more time
- late game one of the arcana cards will pull literally anything u can collect straight next to you every two minutes so saves any walking

I found that the best level for this was Moongollow, using Endless. All 16 items are within a very short distance at the start and so you don't even need to use Arcana card VIII Mad Groove on this level, meaning you can use a Arcana card focusing on health, etc.
If you grab all 16 items then you never need to worry about a specific item showing up and you can then just concentrate on upgrading the weapons.
As you have already said, just avoiding the double weapons (guns, Peacherone and Ebony Wings). I used Knives, Whip, Magic Wand, King Bible, Rune Tracer and Gatti Marri. It was a cakewalk: Level 100 by minute 38.

Here is my full achievement walkthrough for Vampire Survivors. This achievement appears at 18:52 in the video.

As the description says, you need to have six evolved weapons at once. Six weapons is the maximum you can have, so in order to do this, you'll obviously need to be playing as a character whose starting weapon can be evolved. You'll then need to carefully pick your weapons and items such that they all pair up for evolutions, and then level up to around level 50-55 to get all six of your weapons to level 8 so they can be evolved.
This can be done on any stage and with any character whose starting weapon can evolve. I recommend playing on Inlaid Library, and you can play as any character in the first few rows. The best weapons to get to ensure survival long enough to evolve your six weapons are definitely:
