This might initially seem daunting but can pretty easily be done.
*Important note (thanks NativeB1ood) all upgrades from level ups, including support items count as "weapons"
** Also it seems sealed weapons count towards this too (thanks Pedle Zelnip)
First select Queen Sigma (you can use other characters but she is by far the best as she has max default stats)
Then load into a map you are confident on, turn on: Endless, Hurry and Arcanas then load into game.
Immediately choose the VIII - Mad Groove Arcana and pick up a couple of support items (Ideally a Crown). Then use your Re-rolls to focus on getting the Victory Sword to max level as fast as possible, then max out the couple of support items (no more than 4) and pick up whichever Arcanas you prefer.
At this point just proceed to banish Weapons every time you level. You start with 118 banishes (as Queen Sigma) so make sure you finish with no more than 78 (I recommend leaving a cushion if you can).
Once you have banished all options; end the game and the achievement should pop.

You should clarify that the support items count as weapons, I wasn't actually sure when going for this.

This can easily be done with Sigma on any map, but you can speed it up by turning on Hyper and Hurry so enemies come at your fast. Playing with Sigma starts you with 118 Banishes. I first recommend getting at least four or five weapons and accessories so you're good to mow down enemies, but just make sure you don't pick a sixth weapon nor a sixth accessory. Then, once you have most of a build, just start banishing a weapon or accessory every time you level up. There isn't a direct way to track how many you banished during a run, but you can just watch your 118 Banishes go down to 78 to know you've banished 40 things, then quit the run to unlock this achievement.
As a side note, if you have five weapons and five accessories equipped, you will run out of new stuff before reaching 78 Banishes remaining. Fear not. You can still banish equipped stuff too. You won't be able to level it anymore after banishing, but it'll still count for this achievement.
