Very early on, after a meeting at your office and a train ride, you'll find yourself in an alley having a conversation with a character named ???. You'll be presented with two choices :

Don't shoot the mysterious woman in the alley after you get off the subway. You will have to start the game over after this. I could not load my save up as it was deleted after the achievement popped. You could probably get around this by making a copy of the file and replacing it after you get the achievement so you don't have to click through the intro again.
