Second story related achievement. You start off waking up from the end of From here to Eternity. Go down stairs and follow the onscreen prompts. Offhand Weapon Used Stake is in the downstairs of the second building. After you leave the hideout, you will have to fight 4 guys to pass through to the bridge. At the other side of the bridge a cutscene plays and you follow the trail to until you find the bar. Speak with all of the people there and head upstairs.
You meet Doctor Swansea in a cutscene and speak with him, and he explains that there is a monster on the loose somewhere in the area. You return downstairs to question the people there again. They will tell you to explore the docks. Once outside you will search the boats for clues. Once you search the boats, you jump across the docks to the factory and continue through both buildings. The Used Iupara (sawnoff) is in a chest at the far top of the second factory building.
On the other side, follow the trail across the bridge into North Docks. The Scythe is stuck in the wall in the hallway. You go inside the door at the far end to face your first boss. After killing it, it gives you the used bludgeon, interact with the man and a cutscene plays. Doctor Swansea appears in a boat to take you away. Collect the blood sample from the dead boss and interact with Doctor Swansea to get this achievement.

Chapter I
After recovering your presence and attempting to piece together some of your sanity, you will skulk through the dredges of the docks in pursuit of what you think is your Maker. After meeting Doctor Swansea, you'll come to find that your pursuit is leading you further into the western docks instead. The first boss fight comes in the form of facing off against what will be known as a Lesser Skal. The early adoption of the dodge mechanics to rebuke your enemy's advances, then counterattacking, will be key to your survival in this grim landscape, so what better way to force you to get better at it than avoiding the advances. Should you need to and if you have it equipped, use the Stake with Triangle to attempt to stun your enemy (the stun gauge is underneath the health bar) and then feast on his blood. You can then use Autophagy or Blood Spear to retaliate further. Keeping your distance at times, while also letting the enemy go for the first strike, will be vital later on.
After the fight, you'll help Swansea move Mr. Hampton to the boat, and while you recover the blood sample from your foe's corpse, you must depart with Swansea in order for this trophy to pop.
