How to unlock the Just take a bite achievement in Vampyr - Definitive Guide
During Chapter One after taking a ride with Docter Swansea and arriving at the Hospital, head left and you'll encounter two men fighting each other. Speak to the man that stays behind and he will rudely introduce himself as Clay. You'll be given two options during the upcoming tutorial to either EMBRACE or RELEASE him. You'll need to embrace Clay for this achievement and kill him.
NOTE: Be warned, killing Clay will cause the Merciful Release and Not Even Once achievements to be unobtainable so it's recommended that you create a back-up save.
To get both, either disconnect your Xbox from the internet just before encountering Clay, then delete your save file, reconnect to the internet and have the cloud bring it back.. OR.. immediately after interacting with Clay, go back to the main menu and load your game again if it hasn't saved by this point.

It isn't very clear on how to create a back up save, so I just quit out to the main menu after getting this achievement as it didn't save and I rinsed and repeated with the other one and went from there.

When you arrive at the Pembroke Hospital for the first time with doctor Swansea you have to go along the dock and meet Clay. That is the first time when you mesmerise person (game tell you to do it). When you have to choose to release him or kill him you need to kill him in order to get this achievement.
If you want to earn both this achievement and “Merciful Release” you can do it easily without messing about with game saves and replaying isn't necessary. First embrace Clay and you will get this achievement then return to home screen and close the game. When you reload the game you will have the option again to embrace Clay or let him go. Let him go this time and you will unlock the “Merciful Release” achievement.

Can confirm it is possible to do it like this. Kill the dude and as soon as you get the achievement dashboard and quit game.

2) Plug it into your PS4.

4) Make a copy of your save data right as you have departed Swansea's company (you will be aware of the save state based on if the Y appears, as this is the autosave icon in the top right).
5) Load up your game, embrace Clay, wait for the trophy to pop, then return to the Save Data Management.
6) Instead of Saved Data in System Storage, choose Saved Data on USB Storage Device.
7) Overwrite your save with the older version that had been separated, then resume your game.
