How to unlock the Unlife is strange achievement in Vampyr - Definitive Guide

During Chapter 4, you'll be able to access the west-end district. Just north of Ried's mansion, you'll be able to access a building that contains a Pure Water item inside it. However, this area is guarded by a level 27 vampire and if you fail to kill it, the civilian Kimura Tadao will die which doesn't affect much aside from his investigation, however, if you're running a 'Not Even Once' playthrough you must be extremely careful as you'll most likely be under level 20 here.
Now that you have the water, head on back to the hospital and feed the plant. Sleep for 3 nights in a row until the plant grows and interact with it for the achievement/trophy.

It is highly recommended that you make some sort of back-up save as sleeping 3 nights will cause the civilian population to contract illnesses, making the game more difficult.

I just want to clarify something. you dont need to immediately sleep 3 days in a row, right? it's just the plant takes 3 in-game days to finish growing. so after watering it, can't you just continue playing normally? I imagine you'll sleep at least 3 times between chapter 4 and the end of the game.

I can confirm you don't need to immediately sleep 3 days in a row. Just continue the game as normal and after you've slept 3 times, check on the plant.

Yeah, I just slept 3 days so I could get the achievement. That's why I suggested a back-up save.

Does anyone know if you can check on it after the 3rd day? I'm not sure if I've slept 3 times and I can't seem to check it.

After giving the pure water to the plant, you can continue on and play the game normally. You only need to check the plant sometime after 3 nights of sleep and obviously before the point of no return. Chances are, you'll sleep more than enough through natural play in order to level up. No backups or sleep marathons are necessary unless you're impatient.

Fighting that guy was super easy. All you do it lead him away until he looses interest in you, then you attack and jump back to the point he looses interest with you. You can also lead him to nearby enemies and they will take his health down quite a bit.

There is no "backup save" in this game. Just play normally following the Walkthrough and you will get it eventually without risking someone dieing to illness. No need to rush this.

In your room at Pembroke is a dying plant. Looking at it, Reid will comment that he wishes to save it but he needs some water. This can be satisfied if you wait until Chapter IV in West End. One of the side streets, under Sense, will show Kimura Tadao in a nearby building. If you are near the Reid estate, go into the hideout (his room) and use the door leading out, drop down to the fountain plaza, break the barrels/boxes, and then turn left. You should come across a crazed Blood Ekon that is holding Kimura hostage.

In this same little square is a boarded-up building with a ledge you can Shift up to. By clearing the building out, you should be able to come across a Pure water key item which you can then take back to your hideout in Pembroke, water the plant, and let it thrive by sleeping for three days (or just progressing and upgrading yourself whenever you can between now and the end). Once it has fully grown, it will look vibrant and lush, and by interacting with it at this point, you will hear Reid comment on it and then the trophy will unlock.

To get the Unlife is strange achievement, you'll have to acquire pure water which is located in a small office during Chapter 4 [West-End]. You can find the exact location in the video below. Since you can't free roam the map once you complete your playthrough. I highly recommend you to do this before Chapter 6.

When you first arrive in your room in the hospital during the main quest “Take me to a Hospital”, there will be a dying plant on a stool. You will need to obtain the item pure water, which can be gathered in the West-End, which you will explore in Chapter 4. One you get the water, go back to your room in the hospital and give it to the plant. It will take 3 nights for it to grow.

For this achievement you must grow the plant in your personal room at the hospital. First, you need to collect "Pure Water" which can be found in West End, and the earliest you will come here is Chapter 4. After getting the water, you can use it on the plant, then you simply need to sleep for three nights and interact with it again after it has grown. Follow the video below for the location of the water.

For those who are too lazy to watch the video. The water is in the building opposite which we save Kimura. There will be a locker right at the entrance, near the door.

It was strange not to see screenshots with a map in the hint for obtaining this trophy. The marker indicates a clear place on the map where you need to go.

To make it even easier to find the building we need, I took a screenshot of the desired door that we need to go through. Immediately after we entered, we turn to the right and there will be the safe we need with the coveted “Clean Water”.
