After you unlock the second hideout, you gain access to a crafting table (hammer icon).
By killing and looting guards and chests, you find materials to craft upgrades with. When you select a weapon to upgrade, in the top right corner there is a green icon that lets you see how many of the materials you need to complete it.
Once you have enough for your first upgrade, hold A over the icon to craft it and unlock the achievement.

This trophy is technically story related, but I suppose if you wanted to try and avoid it, it isn't mandatory. Unlocked with hideouts, you will find that there are workbenches that you can make cure tonics, disassemble junk you find along the way, and even upgrade your weaponry. This trophy will unlock as you upgrade a weapon, which will probably be one of the first two weapons you come across (the machete, revolver, or stake). Simply upgrade it to the proceeding level to get this trophy.
