Story related and cannot be missed.

Just get to the end of Chapter 7, solve a decently simple puzzle, then watch the final cutscenes and the achievement is yours.

This trophy will pop once you've finished Chapter VII. I won't spoil anything more for you other than I do hope that you didn't have to play through it a second time to get the Not even once trophy, as some of the boss fights are much harder than they need to be because of the under-levelling involved. Perhaps if you satisfy sidequests early on when people are still around, it might be easier. If you've completed the Not Even Once trophy, both this and that will pop at the same time.
