The achievement description is a little misleading/vague. You're not allowed to simply reload a checkpoint, kill all the soldiers, and then wash-rinse-repeat. It takes your personal best at the end of every level and adds that to the total, disregarding any "extra" enemies you may have killed before reloading a save for any reason.
You'll have to be very thorough when going through the levels and killing the enemies. I didn't unlock this achievement until the very last level, around the middle of the mission. Don't miss any chance to kill an enemy, and especially not to stealth kill them. I unlocked the stealth kills two or three guys before unlocking this achievement.

For reference, there are 344 killable enemies in the campaign and the earliest you can unlock this achievement is 5 kills into the last level (assuming you've killed every single enemy in the first 11 levels.)

If you want to get this without any issues just kill every single guard on each level and you should get it in no time. If you're after the assassin achievement as well, those kills count towards this also. It doesn't matter so much about how you kill them just as long as you kill as many guards as you can in any form.
