If you DON'T have the DLC, you need level 30 (5 destiny card slots) and then the Adventurer's Outfit, which adds a 6th slot. If you DO have the DLC, you get a 6th slot at level 55, though you can still get it earlier with the Adventurer's Outfit. The reason the DLC matters is the level cap without DLC is 50, and the DLC ups the cap...to 60 I believe.
In order to get this achievement you need 6 destiny card slots, and you need 6 cards with a divine attribute. Some destiny cards glow, and these either have a divine or wicked attribute. Wicked attribute won't count for this, you need 6 with divine attributes.

For this achievement you need the adventurers outfit which gives you an extra destiny card slot.

You can equip 6 cards on level 55. It does not matter the level or value of the cards as long as they have "The Divine" before its name.
