When you have killed the final boss all but one location are displayed on the map.
Any locations which are locked cannot be entered directly from the map and must be accessed via the door in the connected large area. Any locked doors are slightly grey within the door symbol on the in-level map, opened areas are clear. This is quite subtle but can be used to identify which door you need to enter the locked area.
The final location is the Bottomless Pit which becomes available when you hit level 36. This can be accessed from the map once you are level 36.
You only need to enter the area. You can exit straight back to the Hub as soon as you enter.
There is no specific method of tracking which areas you have entered. I would recommend either completing a challenge or locating a secret in each area as this is displayed on the map and can be used to ensure you have entered said area.

Apart from the story line areas, you will need to "find" the other areas in the game by walking up to their gates to unlock them. Once you enter the area it will count. You also need to enter the "Bottomless Pit" and the two areas.
