It's best to approach it in coop.
Unfortunately you can't jump in to the final boss fight like in main game, this time you'll have to do the full story. You'll be skipping most of it though. Make sure your coop partner completed the DLC at least once, you'll need their teleports. Total time requirement: 30-45 minutes.
As you'll be having all 5 debuffs active, you'll lose health from time to time. Make sure you heal up once you're down to about 25%.
Start by creating your character and passing the tutorial. The only tricky solo fight would be the one against horde of skeletons and succubus while the Warpig fires at you. Stay away from its projectiles and you'll be fine. When asked about the difficulty pick hard and hardcore, the outfit doesn't matter.
You'll land in the pub (or any other main hub, just change the world and go to Motorhead). Join your friend in online. Speak to the bartender and teleport to The World of Wars. Immediately go back to the Pub. Your friend should teleport to Heart of Destruction, leave the area back to The World of Wars and kill the enemy in front of the portal. Once it's clear teleport to them, trigger a conversation, go back to the pub and warp back to your friend. Both of you should enter the Heart of Destruction.
Make sure your objective is to slay the warpig. Let your friend jump down (you should stay at the terrace, close to the fight) and start killing the machine. You should see the health bar at the top, if not, your friend should lure the machine to you, get killed, machine will target you (you're invincible at the terrace) and its health bar should appear. Wait until your friend kills it, make sure the mobs are not there, jump down for the loot and use the teleport to go back to pub.
Level up, pick Hope card. For the other rewards pick whatever you like (chests?). Make sure you equip Kilminster guitar.
Teleport to the Weird West and go back to the pub. Your friend should go to Terminus Est. Warp to them. Make sure your objective is to slay the Orgasmatron. If not, both of you should leave the area and go back to Terminus Est (you can skip the level requirement if you teleport to your partner). Once it's ok, trigger the fight.
This fight is the only real problem. You will be participating so make sure you dodge all the time! Stay away from the action and especially from the train, ask your friend to kill what's after you. Run at the edge of the map. If you're in a dead end, hold
for wicked solo which works like a getaway. Once your friend kills the train (and you didn't die), teleport back to the pub.
Go to the Dark Ages and back to the hub. Your friend should go to the Chalice. Join them and make sure you have to kill the Queen of the Damned. Same as Warpig really, you stay where you are, your partner lure the boss (it can also skip her stages), you have a health bar, they kill the boss. Once she's dead wait until the hallway is clear, teleport down, run to the throne and use the teleport to go back to the pub. Achievement should pop afterwards.
