This is easier to do in co-op but it can be done solo. Start by shooting an enemy with any lightning gun to apply the Electrocuted effect (This effect lasts quite a while). Then use any set of revolvers and use the charged shot to apply Crippled, Bleeding, Weakness and Dazed. You have to do the next part fairly quick as the Dazed effect from the revolvers doesn't last long, but either pause and swap to a guitar to do the Power Slide ability or use the Paralyzed demon power.
This can't be done on a champion as they are immune to Headbang.

to ease things up you should use "wicked" destiny cards (add crippled/bleeding on crit) and demon powers to apply the status - it is less complicated that way.
e.g.
1. drop heartbreaker => weakness & electroluted
2. try to score a revolver crit with bullseye (B) (=> all you need is dazed), or you could use a scythe (Shockwave)
3. use the guitar wicked solo (headbanging) - if its a crit you're set too
I'm not sure if its has to be applied "all at once" or just all of them before the monster dies, I happend to get this on accident while fighting a scorpion earthshaker. while using a guitar and heartbreak with the said wicked destiny cards.

To make things easier you can do this on the Dummies in the Hub. They‘re stationary and cannot die.
