This achievement is story related, and can be unlocked by defeating the boss (Screeching Calamity) in the Defiled Chapel.
The boss fight is basically several of the Wraith type of enemies. One of them will be the Screeching Calamity. All I did for this fight was try to focus on one enemy at a time by luring them away from the others. This made it easier for me, especially when considering the cooldown on the health potions.
