The Fractured Worlds DLC includes 5 areas:
The Fracture - a multi-tier dungeon.
Hanging Gardens of Ea - a single dungeon that changes every day.
Cadaver City - a single dungeon that changes every day.
Amaranthine Autumn - a single dungeon that changes every day.
The Lethean Caves - a single dungeon that changes every day.
This achievement is for the dungeon that changes each day. The story portion of the game will take you to this dungeon twice (once after reaching Tier 10 in Fracture, again after reaching Tier 25). You need to visit the dungeon another 3 times over different days.
The achievement is not tied to your character. If you have multiple characters across different save slots, the total number of visits is accumulative.
Achievement pops as you enter the dungeon for the 5th time.
