How to unlock the The Elder One achievement in Victor Vran - Definitive Guide
Play on casual, bust through story line. Have plenty of potions handy. Stash your favorite weapons off of other playthroughs, and you can use them on this. Pretty simple.
The reason I am posting this is, I died on the second half of the battle with Lord Gabriel, where the screen comes up to choose to Respawn or Return to Hub. I turned my Xbox completely off because I couldn't believe I am an idiot and I died. I returned the next day thinking I would have to do it all over again, I checked that save slot and it said I had no deaths. So I booted it up, beat Gabriel (didn't have a brain fart this time) and the cheevo popped. Not sure if it is because I never "respawned" or what, but this worked for me, so if you die anywhere along the way, try this and hopefully it will work in your favor.
Also to note, my xbox is set to restart when I turn it off, so the game "reboots", you could probably just quit game and have it reload and not have to reboot the whole xbox, although I didn't do it that way so I am not positive on that method. Just want to make it as clear as possible.

I can confirm that as long as you don’t choose respawn it won’t count as a death. I died three or four times on normal and just dashboarded and “quit” the game.

Took me about 2 hours to boost this achievement on casual. I just ran through every zone ignoring all the enemies and focused on the bosses. Only part that required grinding was a zone which required level 10 to access. All I did was jump from zone to zone completing the challenges that rewarded EXP.

The repeter - Upgrade to max.
Finger of Chaos - While equipped : Find 40% more items.
Berserk Aura - Upgrade to max and active all the time.
Vigilante's Outfit - Armor: 70, +10% critical hit chance, Critical hits grant 300% more Overdrive. Normal attacks no longer grant Overdrive. If you max it.
Lucky Rabbit's Foot - Unlimited use. Gives Luck for 5 sec.
Best stat - Negate A Critical Hit.
Best cards - 1*The Trigger - Dodge attack. 2*The Hunter - Point-Blank shot now grants Might in addition to its normal effect. 3*Vampire - More health per hit life saver. *4 The Runner - The faster you are the more you can dodge attacks.
Just go Forward and don't lvl to much because enemies lvl with you. enemies scale with your levels.

Make good use of the Xbox cloud saves. I died more times against Gabriel than I care to admit to his infinite-range totems and purple wave rubbish.
When you die, press the dashboard button , press burger , choose Manage Game, go down to saves and choose to delete your save. Make sure you choose CONSOLE and not cloud/everywhere. Once deleted, press burger and force quit Victor Vran.
When you load up the game again, it will download your save copy from the cloud from the last time it sync'd. It will occasionally sync during play, so make sure when you die you complete this process as soon as possible. If you want to force a sync (like just before a boss fight), make sure you quit the game and start it up again.
Other non-cloud related tips:
Equip two potions so you don't have to wait for a cooldown in order to heal.
Barbarian card resets cooldowns (including potions) when you reach max Overdrive.

This can be done on Casual. Ranged weapons are you best friend. Also, equip two healing potions (or a healing potion and a Medkit). If you happen to die, you should not hit "Respawn" as it will force you to start a new game. Dashboard the game, erase your local save (so it does not sync with the cloud) and restart the game. I would load some other game before reentering Victor Vran just to be sure. Make sure the number of deaths is still 0 on the load menu.

If you have a friend who has already completed it, create a new character and teleport to him. Stand aside while your friend kills the boss.
If you don’t have a friend, then it will be very difficult, since you simply won’t reach the end.
