This trophy can only be earned in Dolores' Chapter.
As you progress through the game, you'll eventually make your way through a Mirror (and a narrow hole in the wall) that leads to a room full of wooden Crosses. There's a narrow passageway leading down the center of the pile. Make your way through and out the other end. When you get out, there will be some stairs on one side and another room of crosses on the other side. Pick up one of the Crosses and head upstairs. When you reach the room with the candles on the large ring, there will be a dimly lit hallway on the right with a hole in the middle of it. Walk towards the hole and you'll automatically put the Cross down.

Doing so unlocks the door at the end of the Hallway. Walk across the very janky bridge and head through the door. You'll come face to the face with the shotty. Interact with it to unlock the trophy.
In order to progress with the story, you need to move the Cross out of the way as you'll need to jump down that hole eventually, just as a small note.

In the basement, next to the storage room and the "house rules", is a cramped closet with a mirror inside. Break the mirror, enter the broken wall, and slide through to the other side. Open the door to see a large mound of large, wooden crosses. Pick up the one straight in front of you to open the path, but don't drop it. Instead, take it with you as you go through and up to the other side.
You'll have to temporarily drop it to get up and out of the hole. Make sure you pick it back up afterwards! Then go up the stairs. Once you get to the circle of candles, move to the right, and go through the open doorway.
Place the cross on the hole in the floor, then use it as a makeshift bridge. Walk across it and enter the next room. Here you'll see the pump action tactical shotgun! Pick it up! It's action time! Achievement pops!
