So I originally posted this guide in the wrong section (Georges Memento Achievement). Here it is in the right spot, and thank you to @The Nerds Club for pointing this out!
All tapes can be found during Chapter 2 - Dolores
Listed in order (if I remember correctly):
Location 1: In the parents bedroom after coming down from the attic, check the end table in the corner of the room next to the cassette player (to the left of the big creepy mirror if you are staring right at it).
Location 2: Once you find your way into the kitchen, Dolores will ask for some tea and the tape is right next to that tea.
Location 3: In the office on the second floor you will find a mirror that you can pass through (the one with the dead George in a chair). "Place" the knife where it belongs and you will get a key from his hand. Take this key to the locked door in the same room (turn to the left of George and its right by bookcase). Enter the room and look to your left on the table. There is a tiny box with a cloud toy and the second cassette.
Location 4: After busting through the wall in the bathroom on the second floor (clock puzzle area) you'll go through a door and down a hall. On the left right when it opens up will be a tiny table with a cordless phone port along with the final cassette tape.

There are 4 Cassette Tapes to pick up, all of which need to be collected as you play through Dolores' Chapter. Their locations are as follows (in roughly the order you'll be picking them up in):
First Tape: When you first jump down into Dolores' bedroom from the attic. On the table with the Cassette Tape Player.

Second Tape: Down in the basement with the mannequin and blue draped cloth, there will be a Mirror you can break that leads to a kitchen/dining area. On the kitchen counter is the Cassette Tape.
Third Tape: This one is somewhat story related as it'll be incredibly hard to miss. When you place the knife back in its rightful place in the Study, you'll get a key leading to a secret room on the far side of the Study (you'll need to move the bookshelf to actually reveal the door). Head inside and open up the small box on the table. Inside is the 'Cloud Toy' (story related) as well as the Cassette Tape.

Fourth Tape: Inside the Bathroom is a Mirror you can break that leads to some long hallways with clocks above them. Go to the first "intersection" and there will be a table on the left with a baby monitor on it. Just next to the monitor is the Tape.
You don't actually need to listen to them for the trophy, you just need to pick them up. If you'd like to listen to them, though, there are two Cassette Tape Players in the house. One in Dolores' bedroom when you drop into it from the attic (only available while actually playing Dolores' Chapter), and one in the Master Bedroom upstairs on the table just next to the window.

Note: This is only possible to get during Dolores' chapter. You will automatically get the "George's memento" achievement as part of this achievement.
1. Drop down from the attic and into the mirror world bedroom, go to the corner with all the numbers written on the walls and papers. On the stand there you'll find an audio cassette recorder and the first cassette.
2. Enter the mirror in the upstairs bathroom. Inside the hall labyrinth, where you have to choose which way to go based on Dolores' secret numbers, the cassette will be on the small stand with the babycall base.
3. Enter the mirror in the upstairs office. Through the "secret door" in the room with the dead body, you'll find a cassette in the same box as the cloud toy.
4. Enter the mirror by the mannequin area in the basement. You're now in the mirror world kitchen, where Dolores asks you for tea. The cassette lies on the counter to your left immediately after entering.
Once you've picked up all four cassettes, the achievement will pop!
