Be aware! This achievement is glitchy. I had to reload an earlier save and make the hot chocolate again to get the achievement.
Edit: I would suggest completing the first chapter before starting on this achievement. There are reports that you are unable to complete it until afterwards.
We are gonna make some hot chocolate! Let's get started! Here's where the ingredients are at:
Hot chocolate packet - In the bottom drawer next to the oven.
Milk - Inside the fridge. Back on the middle shelf.
Now that we have our ingredients let's put them together. Go to the tray next to the microwave and place your milk and hot cocoa packet next to the lonely cup. You will get a cutscene where you place the hot chocolate in the microwave. Next, your hot chocolate will dissappear and go into another dimension. Head up to the boy's bedroom upstairs and you will see a swirling vortex at the end of the room. Walk up to it and a cutscene will play with you enjoying some hot cocoa.

I’ve done it four times already and it won’t show up in the bedroom. Gotta hand it to these PC developers 🤬

Try beating Lucy's chapter first. I was looking through the steam forums and somebody said that spawned the portal for them. If that doesn't work, you can create a new game and go to the kitchen as soon as the game starts. Hope it spawns for you!

This achievement was glitched for me too. Reloaded lot of times previous saves, but it didn't work.
So that's what I did:
Reloaded a previous save and completed the first chapter and the dolore's one (Lucy missing), than went to the kitchen, put the milk first, than the chocolate.
When the mug was in the microwave run as fast as u can to the portal. That's worked for me.

This can be done when you're inside the kitchen AFTER you have completed at least one chapter (the Hot Chocolate Mix won't appear until you do). You can't be in the middle of a Chapter for it, you must be between chapters inside the house (basically in Free Roam).
On the counter, just by the Microwave, is a black mug that, when you interact with it, will tell you that it's sad and empty. Just like my soul. This is the cup we'll be making hot chocolate in. Inside the refrigerator is a carton of Dope Cow Milk and in the bottom drawer by the stove is some Hot Chocolate Mix. Place both items on the platter with the Mug and you'll automatically make some Hot Chocolate. After some sparks and a nifty disappearing act, you'll need to go find your freshly brewed mug of Toasty Chocolate. Head upstairs and into the little boy's room (the room to the left of the bathroom) and on the back wall will be a large spinning void-like portal. In the middle of it will be your drink. Give me hot cocoa or give me death, we say so walk up to the void to pick it up and when the scene is done playing out, the trophy will unlock (there will be a few seconds delay on this trophy, just a heads up).

After playing through one of the three chapters, you should have access to the kitchen. Open up the fridge and pick up the Dope Cow Milk.
Go over to the microwave, and there should be a set of drawers to your left. Open the bottom drawer and pick up the Chocolate Powder.
On top of the counter, next to the microwave, there should be a plate/tray with a cup on it. Use both the Dope Cow Milk and the Chocolate Powder with the plate/tray.
Next, click on it to start a small cutscene where you pour in the Chocolate Powder and Dope Cow Milk into the mug, then place the mug into the microwave.
After the cutscene is done, go out of the kitchen towards the entrance hallway, then go up the stairs. Turn left, and enter the son's room on the right side, which should now be lit in a blue light. Walk straight ahead towards the spiraling, black circle in the opposite end of the room, and a new cutscene will start.
Enjoy your hot chocolate!
