You must obtain the "Negligence" VHS tape from Dolores' Chapter, see (Dwayne's Memento) achievement guide before you're able to follow this guide. Thank you to (LeXXuz) in the comments for your guidance. You must be outside of a chapter. After you pick the tape up, you must play it in the VHS player next to the TV in the lounge, first floor. After this, if you go into the basement next to the TV in a photo frame there is a vinyl and if you place it into the phonograph, vinyl player next to the TV. The wall where you picked up the vinyl will start to crumble, if you walk in and push the shopping trolley, picking up crates of beer. You will eventually reach the end, continue throughout the course and eventually you'll pick up a glass shard. Which is one piece of the Mirror Mask.

That doesn't work that way. At least not in my game. You need to collect the VHS tape "Negligence" first, put it in the VHS and THEN there is a vinyl in the middle picture frame by the TV-Set in the basement.

Sorry yes, I should of added that I meant you had to be out of a chapter, I don't believe you can access the vinyl if you're in a chapter. But yes, you should have to collect a VHS tape. Thank you, LeXXuz, i will add this to my guide.

To get this achievement, you must first locate one of the seven VHS tapes. (See the "Dwayne's memento master" achievement.) Put it into the VHS player to the left of the TV in the living room(not the basement), and the TV will play a short video clip on repeat.
The video clip gives hints to where in the house you must go and what you must do to start the chosen storyline. Each VHS has its own storyline with new areas to play through. At the end of each you will find a small piece of blank metal - a piece of the Mirror Mask.
Obtain any one of these pieces, and the achievement will pop!
Note: The easiest/quickest one to get is probably the Addiction storyline. Play the Addiction VHS, then go to the master bedroom. Where the bed was, you'll see a hole. Go down. Interact with the bed sheet where it seems like someone is sleeping. Go to the TV and sit down. Take a drink every time you're prompted to. At the end you'll get your Mirror Mask piece!
