You can only grab this achievement during Rakan's chapter. After you have been sent to the hospital a second time, you will be chased by a group of faceless beings. Once you have escaped them and went down the stairs, you will come upon more hospital rooms. Don't grab the key in the admissions office or you won't be able to get the tape! Instead, head to the Medical Records Dept. room. Its on the right side of a hallway you go down. There is a payphone close to it. Inside the room will be a tape on a desk that you can play revealing Rakan's mental health and your achievement!

You can only get this trophy during Rakkan's Chapter and, more specifically, during your second visit to the Hospital.
On your second visit, you'll reach a part where you have to crouch (
) through some half-naked beings to grab a Staircase Key on the ground. When you reach this staircase (you'll actually be chased to it so it's hard to miss), go all the way down the stairs and through the double doors by the vending machines at the bottom. Go straight down the hallway, then right when it ends, then take another right when it ends again. Walk forward a bit and some lights ahead of you should flash on. On your right, you'll see a couple of big green curtains. Just past these will be a hallway going right. Take this hallway and immediately on your right will be a slightly open door leading to the Medical Records Dept (there will be some footprints actually leading into it).
