Simply complete the game normally. Continue on through the Path of the Crystal, die as many times as you like, complete World Six and Ending C is yours.
If you completed Ending A first, you can then start a new game, and keep going left at the start of each world to skip the world. All you have to do is beat world five and six normally, and the achievement is yours.
