You'll get this during Mission 1: Clusterfuck.
After your first on-rails shooter part, where you jump from one building to another, you'll kill a couple more enemies and chase the SWAT leader up the stairs as Wesley saying something along the lines of "He'll run out of men eventually." Shortly after that you'll see one enemy come out of a doorway. Kill him and turn around and you should see Kathy banging on the window. Kill her for the achievement.

Can be obtained in the first level, where you play as Wesley and have to chase the SWAT team leader.
After the first quicktime shooting event (where you kill the swat team members as Wesley dives through the apartment room and then out the window.) The game will resume in a partially constructed building, after killing the three enemies ahead of you, go up the stairs and across to the building to your right. Look at the windows on your left once you have reached the other building. In the window that is farthest to the right Kathy will be screaming and banging on the window. Do yourself the favor of putting a bullet through her head.
Achievement Unlcoked.
