Once in the main menu, click B to back out and select Hardcore.
You must win the game and come in the top three on your team.
You can increase your ranking by killing enemies, planting/defusing the bomb, and getting assists.

By chance, do you guys remember how many to start the match?
Edit: Looks like the forum says 10 for 'ranked', I'm guessing that's the same as hardcore.
