You must get a triple kill in versus for this achievement. When you get a successful triple kill a gold text will pop up on your screen saying “triple kill” along with the points for the kills. I suggest Free For All gamemode as you have more enemies and more chances for a triple kill. Team Deathmatch is also a viable option.

Ive twice got Triple Kills with grenades and this hasnt popped so Im going to safely assume it has to be with either your primary or secondary weapon.

Description is a bit misleading. You have to get a triple kill(kill 3 people within a few seconds). Have to be using a gun because grenades or anything else wont work.
