Play free for all and sit in a corner and wait till someone runs by, run behind them and then slide(while sprinting press B) and then kill them while sliding. You will know if it counts when a gold text pops up on the top of the screen saying “slide kill”. Repeat 5 times for the achievement. Beware though this game is glitchy and it took me a few tries for this achievement to register. Free for all is not required for this achievement but a versus match is. I just suggested free for all as you have more targets

No. Just cumulative. You can track your progress in the challenges menu

Killing 5 enemies while sliding is cumulative, It doesn't have to be in one slide and doesn't have to be in one match.
To initiate the slide you need to be sprinting by tapping in
then tap 
, The slide will only last a few seconds so make sure you are ready to make the kill. (You can also look around while sliding)

My advice for this achievement is to play either free for all, or storm. Select medic, as they have shotguns. And just run around, as soon as you see someone, run up to them, slide, and then double tap the shoot button near the end of your slide (as 2 hits with a shotgun is a guarantee kill).
