To obtain a 3 star ranking, complete both secondary objectives on a map that has secondary objectives
To get this to actually unlock, you need to replay a stage and have 3 stars on the overworld afterwards, so either replay a stage that you already had at 3 stars or improve one to 3 stars. For me, it only unlocked after I completed the Fahrii campaign then improved a 2-star rated mission to 3 stars.
It can be obtained in Story mode difficulty (the lowest official difficulty)

I got it after completing the campaign then cleaning up one of the 2-stars that I got. If it also works by repeating a 3 star, great

Can confirm replaying a level you've earned 3 stars on will pop the achievement, even if you don't earn 3 stars on the level the second playthrough.

For the record, just replaying a mission that you've already 3 starred seems to be the ticket. I was able to replay mission 1 without finishing the campaign and it unlocked.
