Story related, you get this one at the end of the mission Forbidden Knowledge. The vortex grenade alone takes out 75% of his health on exterminatus difficulty.

Well there are multiple sections where you fight a Lord of Change in the game. But I believe each time it is required to beat them to progress through the levels.

There are multiple of these throughout the campaign. When they appear you have to beat them to progress, so you can't miss them. First appears towards the end of chapter 1.
