Like it says complete 10 Missions in a row without falling, in this case it means you lose ALL squads, if you do that on the first 10 missions you have started the game on too high difficulty or you rush into combat without any sense to use cover. Play on easy, take it slow and this is a freebie.

Failing a mission is when all of your squads are 'unconscious' (ie. the red hexagon is above their heads and must be revived). Simply complete ten missions without failing one. If you are on the verge of defeat, try to exit the game and start again. Doing so will not count as a defeat.
