The requirements for this achievement are obvious, and any type of mission will count, i.e. campaign, random, tarot, priority assignment, whatever.
The main thing I wanted to mention is that the in-game counter for this achievement, like many, many of the others seems to be wildly inconsistent. The Xbox achievement tracker seems to be accurate and you should unlock the achievement right at 25 missions without dying. The in-game tracker for the corresponding Heroic Deed is far less dependable. I completed well over 30 mission without dying before the Heroic deed showed as completed.

do they need to be consecutive?

I pretty sure the 25 missions do NOT need to be consecutive. I've unlocked this and have died several times, and I'm not very far in the game.
