Completing the Investigation Terra Incognita is required for this. It involves discussions with Omicron Ankh in the command bridge and multiple levels in the Triglav Subsector. This Subsector requires Level 30 to enter.
Once you reach the end of the final map mission you will locate a Psyker, you must decide his fate to end the mission. After choosing the Puritan or Radical judgement for the Psyker the mission ends and the achievement unlocks. I chose the Radical choice and it was in Coop but either choice should work.
