This is a story related achievement, but is still technically missable. You will unlock this achievement upon completion of the Alpha Pariah quest at the end of Season 1, but only if your morality choices put your gauge on the Radical side of the slider.
If you've been following the Puritan path, then you'll unlock this achievement instead:
The Secret Achievement achievement in Warhammer 40,000: Inquisitor - Martyr worth 101 pointsContinue playing to unlock this secret achievement
In other words, you'll need to play the Season 1 campaign twice to get both achievements. Since the achievement is based on your morality choices, there's no opportunity to quit out and replay the mission for the other ending.

I’ve done this multiple times and the achievement just does not work.

I’ll have to try, thank you. I’ll have to run it with crusader again. I did it with psyker and tech
