To find the "Ironman" setting, you'll find it on the front menu when starting a new game, on the right, shown ERE.
The Ironman setting means you cannot save the game at all when playing the game, so any decisions made that you didnt like or any deaths that occur etc will not be able to be reloaded and tried again.
Once selected, you should see an "Achievement Available" light up next to it, this means it's enables and will unlock once completed.
