First chance: Level 1
When: < 30 minutes past prologue
You will be given a popup/prompt when you get near your first case of grenades, it glows blue like the ammo crates.
Directly after this first grenade case is a perfect chance to get this, as three waves of 20-40 small to medium orcs come over the wall to the right. As soon as the first group of orcs in a wave is on the ground throw a grenade right in front of them, the fuses are short and chances are you will knock out 5+ of the small orcs on the first wave.
If you run out of grenades there is a second box behind the barricade to the left; as a last resort, die; you will reload just before the first grenade box.

Exactly where I got it but anytime you get a bunch of Gretchin together it's worth lobbing a frag at them.

Throw a grenade into a group of Orks/Gretchins. You need to kill 5 of them.
A good place for this is at the beginning of chapter 5 with the group of Orks when your squadmate talks about them and the crane.
To get this one, simply lob a grenade into an oncoming group of Orks or Gretchin. As they're almost always in hordes, you're almost garunteed to get this one in the first 5 Chapters.
