This achievement requires you to equip an orange item in every item slot of one character. Purple or Red items do not count. You will likely have the most problems getting 3 orange trinkets.
Try running missions on nightmare and collecting all tomes and grimoires on a mission. This will very likely give you an orange item every run. Also make sure not to equip your class trinket as this will severely reduce the odds of getting a trinket.

Fun fact! Unlikely to happen though. If you equip 2 orange trinkets before reaching level 35 and unlocking the 3rd slot, the achievement will unlock.

Either I'm doing something wrong or they patched the game as I have orange Head, Melee, Ranged, and a single Trinket slot, with the other two trinket slots empty, and the trophy doesn't pop.

They patched a lot this game - maybe again something glitching. Try to equip orange trinkets in all slots. Sorry, can't say more, very long ago played this game.
