You must play and complete missions from the Karak Azgaraz DLC to obtain the Pick. You will want to equip Bardin's trinket to increase the chance that you get a drop for him.
This achievement is either mislabeled or glitchy. You MUST kill a Stormvermin that's part of a Stormvermin pack to get this achievement. I killed probably 100+ regular Stormvermin that were off by themselves and the achievement would not unlock. I then killed one in a Pack and immediately got it.
You must also make sure that you're the only one that damages the rat.

How do you ohk them? I cant kill them in hit with strength potion on easy (have a blue pick, and use charged attack)

You charge it up all the way. You'll do a running attack and you'll kill them in one hit on easy. Make sure to aim for the head.

Here is My Solution for acquiring the Executioners Sword in The Stormdorf DLC ... The same method can be used to acquire the Pick . in the Karak Azgaraz DLC .
Where I mention the word Sword please read Pick ... Wear the trinket for Bardin , the Dwarf character , and play any of the Karak Azgaraz DLC missions ( We found ' Chain of Fire ' the quickest )
Equipping the Executioners sword is very simple but the hardest part is actually acquiring it , so that You can equip it , here is a method that will greatly improve Your odds / chances of getting it .
The Sword is for Markus Kruber , the Empire Soldier character , so equipping the trinket 'Hammer of the Empire ' will give better odds that it appears at the dice screen.
Play with 4 friends , or create a boosting session , and then play either of the 2 Stormdorf DLC missions ( Reaching Out is by far the shortest ) on any difficulty ... Obviously ' Easy ' is the quickest .
However , Do not all play in the same game !
1/ All play a game seperately , keeping in contact thru party chat , then as You reach Olesya's wagon this is what happens ...
2/ Everyone waits at the wagon ( in there own game ) , Unlike other levels it seems that there is no more attacks once You reach the wagon not even from 'specials'
3/ Gamer 1 will then enter his wagon ,finish the game and then throw the dice and keep his fingers crossed that He gets the Sword.
Lets say He DOESN'T get the Sword ...
4/ Gamer 2 , who is still waiting at his wagon , sends a Game invite to Gamer 1 ... They then close the door ,throw the dice and again hope to get the Sword.
Lets say they DO NOT ...
5/ Gamer 3 , who is still waiting at His wagon , invites Gamers 1 & 2 to his game ... They then close the door , throw the dice and again hope to get the Sword.
Lets say they DO NOT ...
6/ Gamer 4 , who is still waiting at his wagon , invites Gamers 1,2 & 3 to his game ... They then close the door , throw the dice and again hope to get the Sword.
If no one gets the Sword start the procedure all over again ( new seperate games ) ... However should someone get the Sword that person will start his own game , immediately , and just wait at the wagon then as the others finish invite them All into the game .. Enter the wagon , close the door , throw the dice and hope that they get the Sword.
As You can see one person will get 4 attempts at throwing the dice , another will get 3 , the next will get 2 and the final person will only get 1 ... However, the next set of games You can change the order of who enters the wagon 1st , 2nd , 3rd & 4th giving everyone the same amount of chances at getting the Sword.

what a great guide worked absolutely perfect 5 of us got the sword in a little over an hour. i would add that in the runs no grimoires should be picked up as it eliminates the 0 dice roll and leaves just 1 to 7 where as the sword or pick can appear in the 0 to 7 dice roll

Once you've acquired the pick, simply play a private easy game and take care of all the ads as you're waiting for the patrol.
Once you hear them, go towards them, Take a strength potion just before you walk up to them and perform one charged attack. Achievement should pop.

For this trophy you are required to have the pickaxe for Bardin, which can be acquired by playing the Karak Azgaraz DLC maps, Khazid Kro, Cursed Rune and Chain of Fire. Any color rarity will do, however mine was achieved by using an orange tier pick.
Once you have the pick, you need to find a Stormvermin patrol, not a single Stormvermin, as apparently the trophy is glitched slightly. Once you find a patrol, you need to kill one of them with just one blow without any of your teammates doing any damage to your target. In order to do that you need to use the charged attack, you should hear a thud sound when it hits if you have charged it sufficiently. Holding the attack for about 2 seconds should suffice, however, waiting for a patrol to pass you first and then attacking them from behind with a charged attack will make this much easier. It does not need to be a headshot, you only need to kill a Stormvermin from a patrol in one hit without any other teammate damaging them. Difficulty does not matter other than how many Stormvermin are in a patrol, higher difficulties mean more tries attempts per patrol but also means enemies will hit harder.
