This is slightly annoying and like most of the other nightmare achievements should be attempted with a party of four.
The least riskiest method is to have one designated sack carrier, and the other three players act as bodyguard (I suggest the soldier/dwarf/witch hunter to do the carrying).
The important things to note are that the player carrying the sack cannot go down, get jumped/hooked by specials, get knocked by a rat ogre. So there is a fair bit of luck involved in this. For example, getting an ogre while nobody is holding a sack is much better than the alternative.
