During the final boss fight, Nurgloth summons minions while he taunts you from out of range. This achievement, which can be done on any difficulty, requires that no one (including bots) kills OR HITS (pushing or staggering) any of these minions, only Nurgloth himself. Easier than it sounds (sorta), you just need to build stamina and block while waiting for Nurgloth to come down off his perch. After he comes down, he may kill some/all of the minions himself (this is ok for the achievement) with his eruptions. Then just focus on killing Nurgloth, rinse and repeat until the end of the level.

From the Fatshark Forums: make sure you turn off any passive abilities that cause stagger (or damage) on mobs when hit, as this passive stagger also counts against the achievement. Basically anything that affects the mob (via damage, stagger, knockdown, stun, push, etc) needs to be turned off for the fight.
