I thought this achievement would’ve been to complete all scenarios and side quests in every area (and it may actually be meant to) however when I completed all locations and quests associated to those locations in Tiltrun only it popped for me.
I struggled with the rat infestations (I went for 100% destroying the towers to maximize alchemy items) so my team of 15 (11 people, 3 horses and a bear) was a mix of Level 6-7 and last thing I did to pop it was buying a border pass in the NE.

Definitely seems to be unlocking early. I got mine after just my first POI in Vertruse.

Same as OP, when I hit the final POI in Tiltrun (buying what sounds like the same border pass), I got the cheevo for the completing the region and this one.
