I do not know what for certain finally triggered the achievement after being stuck at 29/30 points of interests for days.
However, it finally triggered for me when all of the following occured:
-Did both rat nest bounties and did not turn them in since they seem to respawn automatically in Ludern.
•Thought that their respawn would "uncomplete" the point of interest.
-Completed quest "Tomb Raiders" after every other regional quest.
-Completed every unlocked step of Trade Route quests up to that point.
•I do not know for certain if these quests affect regional completion since they're automatically added to every region when unlocked, but this achievement finally triggered after getting the trade routes up to date.
