Do not fulfill the requirements for the 'November Reigns' achievement and side with Angela Deth after resolving the Liberty Buchanon quest. Once you defeat the Patriarch in combat, and trigger the ending, it should unlock.

If you did meet all the requirements for November Reigns then you’ll have to mess something up. What worked for me was reloading a save outside Liberty’s garage and killing all the Gippers in Denver Ruins. I also destroyed their giant statue. It changed the dialogue you have with Angela outside of Yuma and it successfully changed the ending.
