To get this achievement you must go to Little Vegas nightclub in Downtown Colorado Springs. This will happen shortly after you finish up with the Garden of the Gods and get the quest "Big Trouble in Little Vegas". In the club, there are six named prostitutes upstairs in separate bedrooms. Pay them $10 each and you'll catch a different STD from each one. After this, find a pile of snowballs (there's one near the dogs outside the club) and pee on it. Gather some yellow snowballs and keep throwing them in the character with the diseases until you get Denver Rot, the last STD needed with a 5% chance to apply.

To add, these all must be active at the same time. All of them give buffs except Denver rot. There is an infinite amount of snowballs, so pick several up to save time. Equip the snowball as a usable item. You have a limited time before the positive ones wear off, but Rot is there until you cure. The time is over 15 mins (1000+ seconds) so not stressful to rush. I haven't found a limit to status effects.

Unless you have a million Yellow Snowballs you should Quicksave, equip and throw your dozen (or whatever) Yellow Snowballs and see if you get Denver Rot. Each Yellow Snowball has a small chance to apply each possible debuff so it may take a couple of reloads to get Denver Rot.
