After getting access to the Ghost Gang Garage during the main dlc questline, you can find a talking-toilet-computer in the north side of the garage. Talk to it and after some dialogue, make sure to pass a level 9 Kiss Ass or Mechanics skill check. Then choose the option "mind if we make a pit stop?". The achievement should unlock.

For some reason I can't get the door to 'spawn' any idea why? Awareness is maxed :/

Which door are you talking about? Maybe something changed with an update? This solution was writing on first release of the dlc.

Secret Head is a hightech supercomputer toilet located in the far north of the Ghost Garage which is accessed as part of the main storyline.
After a few dialogue options and a level 9 skill check (Kiss Ass or Mechanics) you will eventually be given the dialogue option "mind if we make a pit stop?" which will unlock the achievement.
