You'll get this when you're escaping from the Temple of Blood while retrieving the Blood Staff.
After the chessboard portion of the dungeon you'll be dropped into a lake of blood. The mission objective is to the north, but you can escape to the south before fulfilling it if you'd like. You'll need to leave via the south exit in any event, though.
Once you leave there will be a small cutscene and you'll need to give the man launch codes to open the missile door. The code was found on the chairs in the Temple of Blood - the correct launch code is "MOTEKIM".

You will come across a room with a large missile and a man standing at the control console for it after playing the chess game and getting the Bloodstaff from the blood lake room in the Temple of Blood. Tell this man "motekim" to unlock this achievement.
