You'll get the Bloodstaff as part of the questline in Needles. There will be loads of useless fake Bloodstaffs dropped by all of the enemies. Just ignore them.
The real Bloodstaff is found on the corpse of the Blood Priest on the central island in the lake of blood underneath of the Temple of Blood. Enter the Temple of Blood and make your way through until you kill him, loot it and escape.
Once you have the Bloodstaff do not turn it in to the Mushroom Cloud Church for your reward. Instead you have to verify it for the achievement.
This is done by going to the western section of Downtown. There will be a guarded building along the north strip with a basement. Enter the basement and there will be a Bloodstaff verification device on the right. Use the Bloodstaff on it to get the achievement.
Afterwards, turn it in for loot!

The, Pants, Party
In Needles, you will find this device in the basement of the building directly across from the hot dog stand and next to the howitzer. The real Bloodstaff is located in the Temple of Blood. Go through the chess game (solution in the walkthrough linked in the Road Map), then swim across the blood lake and kill everyone to obtain it. Once you have it, use it on the machine back in the basement to unlock this achievement.
