Here is a solution to get magic smoke AND Black hat trick. In Act 2 there will be a mission called Uninvited in which you have to prepare an Ambush for Iraq's men. BEFORE you hack the security terminal to start the countdown until they arrive, plant an IED (not a proximity IED a regular IED) on the tire tracks to the right of the computer (while facing it). The tire tracks will appear as a red square while looking at the ground. Once you plant the IED hide behind the shutter in a spot where you cant be seen, but can still press the button to trigger the IED.
Once the enemy vehicle is over the IED trigger focus and detonate the IED. This will net you the 2 achievements. If you somehow fail to get it just restart checkpoint. Much easier than doing it on criminal convoy.
*Note if you are still having trouble with the focus buy all the upgrades for it and/or complete any 10 levels of chess to unlock a focus upgrade.

While all of the above solutions are wonderful, I would just like to add some simple advice that will make everything a TON easier. Craft as many MEDS as you can. Have them equipped when you're going for the focus kills and when you see your focus running low, tap the RB to refill your bar. With 5 meds, you should easily get this achievement no problem!

This is how I did it, right or wrong!
I'd advanced to Act II in the game and had completed numerous side missions in order to get the required skill points for this (It may not take this long - depending on how you play the game).
Upgrade the following skills (when you have the skill points): -
Combat > Improved Focus > Slowed Focus.
Crafted Items > Lure > Focus Boost.
To craft "Focus Boost" You can either scavenge "Pills" from cars you jack (and other random places, "City Hotspots" - if people are being generous!), or buy them from Pharmacy's.
Once you have all of the above I recommend doing a "Criminal Convoy" or "Gang Hideout" mission (You should avoid the killing "Takedown" target however!).
I did this on a Convoy mission - you can block a road off and then wait for enemies to group up.
Make sure you equip "Focus Boost" to your RB. Once enough (4) enemies are in range, enter "Focus Mode" by clicking the Right Stick - go for head shots, keep track of your "Focus" - if/when it's about to run out press RB to get the "Focus Boost" and just go Rambo!
Edit: Of course you can do this using explosives (IED, Grenade, Grenade Launcher) or environmental items.

Your goal is to have at least 5-8 enemies on your screen at any point while having a good rated assault rifle. Pop out, activate focus, and kill 4+ enemies in one instance of focus to gain the achievement.

This is a mix of other solutions. But is very easy. Go to a convoy mission where you have to kill the enemies. Call the attention of the cars, so when they stop, equip the GL-94, use the Focus and just explode two cars. Te achievement will pop-up.

It's not the quickest method but the last mission in the Weapons Trading Investigation has a place where there are always 6 enemies huddled together.
When the mission begins, head up the stairs and kill the three guards patrolling the upper floor, from there look down to the centre of the map and you'll see six guards stood in a circle talking. Equip focus and blow at least four of them away for the achievement.
If you fail, simply restart the mission and it'll spawn you at the foot of the stairs you just climbed.

This guide will help you get the Magic Smoke achievement outside of the campaign, and without killing cops. Start the Criminal Convoy sidequest called "Militia Mile" in Mad Mile. This convoy sidequest allows you to kill the target car, which is important because there will be a very large and indiscriminate explosion. 
Basically, line the street with IEDs, block the road, then hit the middle (target) car with a grenade launcher while focused. I used 4 IEDs per incoming lane. Set them close enough to each other that they are in each other's blast radius. You don't need to use your phone to trigger them, since the grenade launcher will set them all off. Of course, having fully upgraded focus in the skill tree, and the semi-auto grenade launcher, helps.

After a lengthy cut scene you have to escape your burning apartment and protect Clara, head to the right of the wall and dispose of the enemies on the roof. Walk around to the left and dispatch the men running towards you, bye this point you should hear an audio que of a reinforcement request.
At this point equip the grenade launcher and look over the balcony towards the center of the carpark where a car is just pulling up, there are 4 men in this car. Engage focus and fire a couple grenades at the car before the men exit for an easy achievement.
No upgrades required.

I got this in 4 easy steps:
1 - beefing up my Focus as high as possible,
2 - buying the GL-94 pump-action grenade launcher
3 - killed a couple of civilians to get police attention
4 - once 2 cop cars arrived (Each cop car carries 2 officers) and stopped near one another, I blew them up.
I recommend the GL-94 because you don't have to reload after each shot, otherwise you wont be able to reload a semi automatic launcher fast enough before the focus ends, even with it cranked up to maximum.

I found the easiest way to get this trophy was to reach the last mission of act 4 I believe where you are in a graveyard, after the cutscene go straight into focus and there will be 4+ enemies on screen pick them off and the trophy will be yours.

This should be the number one solution on this Trophy. I came here to post this solution, but you beat me to it.
The Focus time is incredibly long during this scene.
There are 6~7 enemies, and if you have the ACR assault rifle, it is much easier to lay them down. Best to switch to the weapon BEFORE visiting Clare's location, as the Focus time auto-triggers (as you noted) and switching to any weapon wastes precious seconds.

Uma dica para essa conquista, é na última missao do ato 4, pois você irá ter vários inimigos na sua frente para atirar, e se você melhorou o foco facilita ainda mais, (Alerta: Spoleirs) o guia é: depois de ter dado um fim no Lucky Quinn, o Damiem irá fazer uma ligação, dizendo que um dos seus parceiros esta em perigo, va até o local onde ele fala para você ir, que esta marcado no mapa, entre no local e, lá irá aparecer bastante inimigo, e seu foco será ativado, atire na cabeça de 4 inimigos e depois irá aparecer a conquista.

Opportunities for this to happen will be many, many times throughout the game; especially in Criminal Convoys and Gang Hideouts. The best way to prepare for this is to visit a coffee shop/bar/restaurant to buy a drink that will give you four additional units of Focus on top of what you have. Also be sure to unlock all the skills in the Focus tree, as well as the Maximized Focus skill by clearing all 10 stages in a Chess game with three stars (Path is by far the easiest).
It's always best to try to get four enemies as close to each other as possible, using Lures and environmental distractions will definitely help for this. Use one of the better assault rifles (preferably ACR) or a shotgun if you're going to be extremely close to the enemy. Aim for the head as most enemies will take a few shots to the chest to drop (especially if they're wearing armor) but one headshot will do them in.
It's always best to try to get four enemies as close to each other as possible, using Lures and environmental distractions will definitely help for this. So I was struggling to get this trophy using the above methods as I was finding I was getting killed before getting all the kills required.
The way I found to almost cheat this trophy was during a gang hideout mission I ran a CTos scan to highlight all my enemies and then snuck near the area most populated with enemies. I then threw down a few IEDs, activated focus then detonated all the explosives.
The trophy popped for me the first time I tried using this method.

Get the focus upgrade skills and the grenade launcher and do a criminal convoy. Create a roadblock with your car and wait for two non-target vehicles to be in range. Hit the focus jump out and shoot the vehicles. Low skill and chance required.
